Documents needed for Planning:
Many people have assets that pass outside their probate estate (their Will). To evaluate and work with you regarding your complete estate plan, we will need copies of the following:
- A photocopy of the Articles of Organization, Operating Agreement, Partnership Agreement or Subscription Agreement for any corporation, limited liability company, partnership, limited partnership interest or family business you own;
- A blank change of beneficiary form for each of your retirement accounts;
- A recent statement for each annuity contract you own;
- A blank change of beneficiary form for each annuity contract;
- A photocopy of a recent annual statement for each life insurance policy you own on your own life or on anyone else (such as children or grandchildren);
- A blank change of beneficiary form for any employer provided life insurance; and
- A blank change of beneficiary form for each life insurance policy you own on your own life or on anyone else (other than employer provided life insurance).
In most cases, we will need backup documentation to do any significant public benefits planning. Although it is not necessary to bring all of these documents to the initial meeting, the following is a list of most of the documents we will need if you retain us:
