Situations you need to deal with may come in the form of chronic health conditions that impact your ability to complete activities of daily living. When that happens, not only do these conditions impact your own hopes, dreams and goals, but they can also impact those around you. This is where we come in. We help families (individuals who are Elders, who have special needs and caregivers) find resources and plan to get the right care at the right time. We are not health care providers, but we understand how the health care system works, how to solve financing problems, how to access care and how to enforce your right to have "quality care." Our ultimate goal is to give you the answers you need to stay home (if you can) or to live life to the fullest in the most home-like setting possible.

In our practice, Elder Care Coordinators work under the supervision of an elder law attorney. We will guide you through the legal issues and counsel you regarding other legal issues of importance, such as putting advanced directives in place, trusts, and eligibility for benefits programs. We work with families all along the long-term care continuum. Although we do not provide any hands-on care, our Care Coordinators are instrumental in assisting clients by locating resources and working with the attorney to monitor the client's situation. As a team, we assist our clients in planning for the future, including possible long-term care needs. We assist clients by educating them regarding rights and resources as they seek assisted living care and nursing home care. Our goal is to transform you into an educated, informed and empowered consumer. Then, we help you enforce your rights.
